The priority of radiology methods in verification of thyroid disease

Purpose. Multi-parametric ultrasonic examination provides information with a high degree of accuracy evidencing changes in the thyroid gland. This clinical observation focuses on the priority given to results of diagnostic radiology methods: multi-parametric ultrasound examination using the newest technologies and radionuclide scintigraphy based on stratification signs when choosing disease management for patients with thyroid oncopathology.

Metrials and methods. The article presents a clinical case of thyroid cancer diagnosis in a female patient aged 57 who presented with complaints for a space-occupying mass in the neck area. A multi-parametric ultrasound examination of the thyroid gland and regional lymph nodes was performed in B-mode, in CDI and energy Doppler mapping modes, compressive elastography, ARFI as well as radionuclide scintigraphy of the thyroid gland.

Results. Despite the fact that findings of laboratory analysis and fine-needle aspiration biopsy showed changes characteristic of benign disease – chronic autoimmune thyroiditis, multi-parametric ultrasound and radionuclide scintigraphy findings indicated the likelihood of a malignant process. The conclusion of postoperative anatomic pathology report confirmed the presence of papillary thyroid cancer with metastasis to the lymph nodes in the patient.

Conclusion. Combined application of modern methods and modes of multi-parametric ultrasound examination and radionuclide scintigraphy significantly improves timely differential diagnosis of thyroid oncopathology and planning of the patient’s further treatment tactics.
