The role of dynamic contrast-enhanced magnetic resonance imaging in planning the surgical treatment of uterine anomalies.

Рurpose. To examine blood flow parameters in symmetric uterine anomalies in planning of surgical treatment and to evaluate its effectiveness using dynamic contrast-enhanced magnetic resonance imaging (DCE MRI). Materials and methods. In the Department of operative gynecology of V.I. Kulakov Research Center of Obstetrics, Gynecology and Perinatology for the period 2015 - 2017 a total of 213 patients were examined and 175 patients were operated with symmetric uterine anomalies, age range – 18-45 years old. Dynamic contrast-enhanced magnetic resonance imaging was performed in 42 patients with symmetrical anomalies of the uterus to determine the tactics of surgical treatment and 30 patients with symmetric uterine anomalies with realised reproductive function as the comparison group, which was not carried out surgical treatment. Results. According to the DCE MRI patients with intrauterine septum were identified into 2 groups: 1 group – patients without disorders of blood flow in intrauterine septum, which in 86% of successfully carrying the pregnancy; group 2 – patients with reduced blood flow, which 84% have reproductive losses in the anamnesis (р<0,01). In patients with the uterus doubling in 85% of cases there was asymmetric blood flow. Conclusion. Method of DCE MRI allows to study the characteristics of blood flow in different areas of interest in myometrium and rudimental structures. Depending on the in-dicators, to choose the tactics of surgical treatment and pregnancy. It is necessary to provide surgical treatment in patients with reduced blood flow during intrauterine septum, aimed at eliminating areas of disturbed blood flow, preventing reproductive losses.
