The case of late diagnosis of congenital right transposition of the aortic arch resulted in great dif-ficulties in the surgical treatment of bronchial fistula after left pneumonectomy.

Purpose. This case describes the role of radiology diagnostic methods in patients with congeni-tal right transposition of the aortic arch. Due to the underestimation of radiology data, con-genital right transposition of the aortic arch was not detected before surgery, which led to the selection of inadequate surgical access and technical difficulties that were successfully over-come.
