Role of CT angiography in the assessment of extracranial aneurysms of the carotid arteries (se-ries of rage cases).

Purpose. To analyze possibilities of computed tomography angiography (CTA) in assessment of rare vascular pathology – extracranial carotid aneurysms.

Materials and methods. For 2015-2018 960 patients with carotid disease were examined in CT and MRI department at Moscow Regional Research Clinical Institute named after M.F. Vladimirsky (MONIKI), 10 aneurysms (1,04%) of extracranial segment of carotid arteries were diagnosed. Examination protocol included an ultrasound scan (GE Vivid 7) and a subsequent CTA with non-contrast study and two contrast phases (Philips iCT 256 slice) with a bolus injection of 50 ml of a non-ionic iodine-containing contrast media at a rate of 4–5 ml / second.

Results. Aneurysms of the internal carotid arteries (ICA) were detected in 9 cases, aneurysm of the common carotid artery was noted in 1 case. Signs of complicated aneurysm were determined in 6 patients.

A combination of carotid aneurysms with other carotid pathology was detected in 7 cases (pathological deformity - in 6 patients, occlusive lesion in 1 patient).

It was shown that targeted ultrasound examination was not able to verify aneurysm in 3 patients of 10. The changes identified by CTA (state of the walls and lumen of the aneurysms) were confirmed intraoperatively. Computed tomography in comparison with ultrasound examination provided more information about aneurysm location and the state of its walls.

Conclusions. CTA is high informative method in assessment of extracranial carotid artery aneurysms and helps to optimize surgery planning for patients with such pathology.
